Jay Wolkoff, MS, CNS, CD-N

 
 

Jay is passionate about providing individualized and comprehensive dietary and lifestyle support to his clients. His unique approach towards health effortlessly weaves together his background in evidence-based nutrition and the emerging sciences, clinical herbalism, appreciation for mind-body medicine, passion for organic gardening, and love of the outdoors.

He currently holds an MS in Human Nutrition from the University of Bridgeport (2015), and a BS in Nutrition & Dietetics from the University of New Haven (2012). Between 2017 - 2019 he completed nearly 2 years of studies at David Winston’s Center for Herbal Studies. Jay is a co-founder and the primary administrator for the “Functional Nutrition Forum – Licensed Professionals Only” community on Facebook which currently supports nearly 5,000 healthcare providers. This is in sharp contrast from his original BA in Accounting from UMass Amherst (2002) and the many years spent traveling and searching for the ideal vocation when he realized that accounting was not the ideal fit.

Jay is credentialed as a Certified Nutrition Specialist,® (CNS®). This is the most advanced certification for personalized nutrition practitioners. Jay is recognized by the State of Connecticut as a Certified Dietitian – Nutritionist.

Getting Started

“Your Path to Wellness” begins with an initial consultation where all aspects of your health & dietary history are reviewed including the use of herbs, supplements, and medications along with past lab work. You will have a thorough questionnaire & intake form to complete ahead of time that often reveals chronic health challenges that have gone on for so long as to feel normal. Whereas to the trained practitioners they often provide clues to identifying “root causes” for complex health challenges and a place to begin your health journey. Generally speaking, the first session will last between 1 hour and 1 ½ hours depending upon how much needs to be shared and discussed.  

A week is suggested in between the initial consultation and follow-up session. This allows for the appropriate research and consideration to go into creating a unique plan that is going to work for you as you begin “Your Path to Wellness.” This may include dietary & lifestyle recommendations, along with the targeted use of supplements, and additional labs to be done.

Follow-up sessions will be spend supporting you along your path and making the appropriate revisions as needed.

Insurance

Jay is currently “in-network” with Connecticut carriers including Aetna, Anthem BC/BS, Cigna, Connecticare, Harvard Pilgrim, and Medicare. He can provide a “superbill” for all “out-of-network” plans for possible reimbursement.

Most Insurance Plans are currently reimbursing Telehealth Visits at the same rate as In-Person Sessions. Some clients prefer to meet in person for the first visit before switching to telehealth due to its convenience. Generally speaking, most insurance plans will fully cover anywhere from three sessions to unlimited depending upon medical necessity. However sometimes a co-pay is required or deductible must be met. Please reach out to the office for further additional questions in regards to working with your plan.
